THE JOB
Are you a dedicated and compassionate Practice Nurse eager for a role that combines traditional nursing with an exciting opportunity in care coordination? If so, we have a unique and rewarding position just for you!
We are looking for an experienced Practice Nurse to join our teams at the Swingbridge and Welby Practices, based in Grantham and Bottesford, starting this November. This full-time role offers a dynamic blend of responsibilities: 1.5 days per week dedicated to care coordination and 3.5 days per week focused on practice nursing.
Position Overview:
Care Coordination (1.5 days per week): Take on a pivotal role in managing and coordinating care for patients, ensuring smooth communication and comprehensive support. You will work closely with the Integrated Neighbourhood Team and Primary Care Network to proactively engage with patients at risk of hospital admission or health deterioration. Your responsibilities will include liaising with Home First Teams, supporting care home residents, and being a key contact within the GP Practice environment.
Practice Nursing (3.5 days per week): Provide routine nursing care, including routine immunisation schedules, cervical screening, contraception reviews and family planning, travel vaccinations, chronic disease management, dementia reviews, cancer care reviews, health promotion and disease management.

Be part of a collaborative team including doctors, Advanced Nurse Practitioners, Nurse Associates, HCAs, Care Coordinators, and a skilled administrative team.
Contribute to proactive case management and patient support within our vibrant communities, located just 15 minutes apart.
Hours: 37.5 hours per week, Monday to Friday.

WHOM ARE WE LOOKING FOR?
- NMC Registered
- Qualified Nurse
- Good understanding of QOF, Enhanced Services and CQC/NICE guidelines.
- Good IT, organisational, communication and clinical skills
- Experience of working within a nursing team essential
- Experience of working in primary care desirable
- Experience of working in a GP practice desirable
- Post subject to DBS check
