Additional needs from Cancer Pre-Diagnosis Team

The ULHT Cancer Pre-Diagnosis Clinical Nurse Specialist Team support people on a suspected cancer pathway who have additional needs. These can be physical, social, socioeconomic or mental health needs.

For anyone identified as requiring additional support, a referral can be made by ticking the ‘Does this patient have any additional needs that require special support through the diagnostic process?’ box on the suspected cancer (2 week wait) forms.

If you are unsure whether a person requires additional support, please refer, and the Pre-diagnostic team CNS will triage the person to confirm whether they require their input.

Please see the attached for confirmation of where to tick on the referral form.
